Rodents and roaches: Restaurant closures, inspections in San Bernardino County, April 23-29

Here are the restaurants and other food facilities that San Bernardino County health inspectors temporarily shut down because of imminent health hazards between April 23 and 29, 2021. If no reopening date is mentioned, the agency had not listed that facility as reopened as of this publication.

One Stop Market, 14518 Valley Blvd., Fontana (partial closure)

Closed: April 26

Grade: Not graded

Reason: Rodent infestation. The inspector saw a live rodent on the floor between beverage cases and an excess amount of droppings in the same area. The store could still sell prepackaged food but was told to stop selling prepared food and to shut off the slushy machine.

Well Being Tofu House, 15024 Bear Valley Road Suites G & H, Victorville

Closed: April 26

Grade: 84/B

Reason: Cockroach infestation. The inspector found an excessive number of live and dead roaches, including in the food-prep sink, on the hand sink faucet, on and around plates and pans and on multiple sticky traps. There was one other critical violation, for raw ribs being kept out at room temperature.

Non-closure inspections of note

Here are selected inspections at facilities that weren’t closed but had other significant issues.

Marquis, at 1036 W. Highland Ave. in San Bernardino, was inspected April 28 and received a grade of 85/B. Among the nine violations, none of which was deemed critical, some cheese wasn’t being kept cold enough, the restroom sinks didn’t have hot water and some other sinks didn’t have cold water. The owner was also told to cease and desist the remodeling of the bar and restrooms, which had begun without health department approval. This was the restaurant’s third consecutive B grade on inspections since 2019.

Baja Fresh, at 929 N. Milliken Ave. Suite 103 in Ontario, was inspected April 28 and received a grade of 84/B with two critical violations. A dozen containers of food were at unsafe temperatures in two refrigerated units that weren’t able to keep cold. Also, an employee didn’t wash hands after handling ammonia sanitizer.

Red Devil Pizza, at 907 W. Foothill Blvd. in Upland, was inspected April 27 and received a grade of 86/B. Among the nine violations, none of which was considered critical, the inspector found 22 dead roaches, mostly in a storage cabinet and under sinks; there was black mold-like growth in the ice machine (not touching ice); and a few items of food were at unsafe temperatures in a cooler that wasn’t quite cold enough. The inspector returned the next day and found two roaches, at least one of which was dead.

Mama Maria’s Grill, at 4204 Phelan Road in Phelan, was inspected April 26 and received a grade of 82/B with one critical violation, for some food being left out at room temperature. Among the 10 lesser violations, the operator said the practice is to leave cooked beans out to cool at room temperature for four hours at a time, shrimp was being thawed unsafely and the hot water had been turned off to the handwashing sink. Four of the restaurant’s five inspections since 2018 have resulted in B grades, records show.

Chophouse 363, at 6939 Schaefer Ave. in Chino, which had been inspected April 16 and received an 81/B, was visited again April 23 in response to two complaints that it was not displaying its B grade card. One complaint said the card was removed immediately after the inspection. Restaurants are required to display their latest letter grade; they can request a reinspection if they want to try for a better one. The manager got the card and posted it back in a window.

National Buffet, at 16920 Slover Ave. in Fontana, was inspected April 22 and received a grade of 78/C with one critical violation. Several containers of food at the self-service line — which isn’t allowed under current COVID-19 orders — weren’t being kept hot enough. Among the 12 lesser violations, both handwashing sinks were blocked, employees weren’t keeping track of how long sushi and rice were kept at room temperature and there was a non-commercial refrigerator. The restaurant has received three B or C grades since 2018 and was temporarily closed in October 2019 for not having hot water.

Updates from past weeks

Coco’s Bakery, at 1140 W. Colton Ave. in Redlands. which was closed April 20 because of a rodent infestation, was permitted to reopen April 23. Pest control had visited, the facility was cleaned and gaps and holes where rodents could enter had been sealed.

This list is published online on Fridays. Any updates as restaurants are reopened will be included in next week’s list.

All food facilities in the county are routinely inspected to ensure they meet health codes. A facility loses four points for each critical violation and one to three points for minor violations. An A grade (90 to 100 points) is considered “generally superior,” a B grade (80 to 89) is “generally acceptable” and a C grade (70 to 79) is “generally unacceptable” and requires a follow-up inspection. A facility will be temporarily closed if it scores below 70 or has a critical violation that can’t be corrected immediately.
